Mailing a Request

The procedures below should be followed when making a formal request under the Access to Information Act for records pertaining to Infrastructure Canada or the Privacy Act for personal information about you.

Step 1

Obtain an Access to Information Request form or a Personal Information Request Form from the Treasury Board of Canada Secretariat website.

A letter may be sent instead. If a letter is submitted instead of the request form, the following information must be included:

  • a statement that the information is being requested under the Access to Information Act or the Privacy Act;
  • a description of the records requested (be as specific as possible);
  • the name, street address, city or town, province or territory, postal code, telephone number, email address and signature of the applicant; and
  • the date of the request.

Step 2

Enclose the $5 application fee payment (access to information requests only). Cheques and money orders must be made payable to the Receiver General for Canada.

Step 3

Send the completed form or the letter to the following address:

Access to Information and Privacy Coordinator
180 Kent Street, 11th Floor
Ottawa, Ontario  K1P 0B6
